Insurance Procedures
- All financial arrangements are made before treatment is initiated.
- All emergency dental treatment or treatment performed without previous financial arrangements must be paid for at the time treatment is rendered.
- We do accept insurance assignments. Please review your insurance coverage and limitations and bring your card and any necessary forms with you. Insurance benefits vary from one employer and plan to another.
- Your dental coverage is a contract between you and your insurance company. Benefits are negotiated and set up by your employer.
- We will fully cooperate in expediting your claim; however, you are ultimately responsible for payment of your account.
- We accept Visa, MasterCard and Discover for your convenience. Any balance over 60 days is subject to a finance charge of 1.5% per month.
- At this time we do not participate in any HMO dental plans. We are preferred providers for several PPO dental plans. Please call us to verify acceptance of your plan.
